About you
As you want a stranger to read it.
Self-declared, not a medical record. Leave it as Not set if you are unsure.

Health notes
Keep it short: allergies, regular medicines, anything a doctor must know first.
Who to call
10-digit mobile, or the STD code and number for a landline.
10-digit mobile, or the STD code and number for a landline.
Optional extras
Only what a doctor must know first, for example diabetes, epilepsy, a pacemaker or blood thinners. Leave it empty if you prefer.

Also save these details in your phone's own Medical ID (iPhone) or in the Safety app's Medical information (Android). On Android you can choose to show them on the locked screen. With a third contact, each contact gets one line on the card.

Emergency numbers on the card

The last five (181, 1098, 1033, 139, 102) are optional and off until you tick them. Ambulance numbers vary by state.

Wallet size: 85.6 by 54 mm, the size of a bank card. Print at 100 per cent scale.

9:41
Your lock screen. Your phone adds the clock and notifications.

Lock-screen pictures: 1080 by 2400 pixels, or 1170 by 2532 for tall phones. Wallet pictures: 85.6 by 54 mm at 300 dpi, front and back. If you have typed nothing, the print and the wallet pictures are a blank white copy with dark-grey writing lines and cut marks; the print also has a fold line, and its back is upside down on purpose so it reads the right way up once folded.

Back to the form

If you cannot speak for yourself, a card in your wallet or a note on your lock screen can speak for you. This tool draws a wallet-size card with your name, blood group, allergies or medicines and two people to call, along with India's key emergency numbers.

Everything is drawn inside your own browser. We never see what you type, and nothing is saved when you close the page.

The numbers come from our guide Emergency Numbers in India: 112, 101, 108 and Which Number to Call, which checked each one against a government or official source in October 2026.

Common questions

Is my information safe?

Yes. The card is drawn in your browser. Nothing you type is sent to us, and we do not save it in your browser either. When you close or reload the page, it is gone. We do count visits to the site with Google Analytics, and the page translator is Google's, so switch it off while you type personal details.

Which emergency numbers are on the card?

112 for any emergency, 101 for fire, 108 for an ambulance, 100 for police, 1930 for online financial fraud and 1906 for an LPG gas leak. They come from our emergency numbers guide, which checked each one against an official source in October 2026. You can leave any of them off. You can also add 181 (women), 1098 (child), 1033 (national highways), 139 (rail) and 102 (ambulance for mothers and children), which are off until you tick them. Ambulance numbers vary by state.

Should I put medical details on a phone lock screen?

Only what you are comfortable with. Anyone who picks up your phone can read a lock-screen picture. If you prefer, print the wallet card and keep the lock-screen picture to a name and a contact.

What size is the printed card?

85.6 by 54 millimetres, the same size as a bank card. Print the wallet-size picture at 100 per cent scale, with no fit-to-page option, then cut it out and fold it into your wallet. If you have typed nothing, the print and the picture are a blank white copy with dark-grey writing lines, cut marks and a fold line, to fill in by hand.

Will 112 work with no SIM or balance?

Do not count on it. We could not find an Indian rule that guarantees it. Keep at least one phone in the family with an active SIM and keep it charged.
